Before we begun filming for each day, my group and I printed off template sheets of consent forms to give to EVERYONE who were on camera - so that we have prove for permission to use them for our documentary. This was done by hand where we filmed the consent forms as much as we could particularly for our main characters (such as Jake).
However for day 2 of filming (the interviews), we only wrote down the numbers, names and addresses of the interviewees, as it would be unlikely that we would need to contact them in the future however was necessary so that we have their permission.
Below are 3 examples of the consent forms which we done for each individual, however these are the most important 3 as they appear in our documentary more than other characters. We decided to show Jake's (as he is the main character), Barney, and the busker, Zak (he was third as we don't really have any other characters whom appear in the film as much as Barney and Jake.
I have blurred all personal information so that it isn't publicised online. I then print screened each consent form after theyve been blurred.
